Support Practitioner (Langley Mill)

Job Title: Support Practitioner
Job Hours: Full Time
Working Hours: 36-48 per week

We are currently looking to grow our team of support practitioners.

Langley Homes is a specialist service for vulnerable young adult and adult care service with Prader- Willi Syndrome, Autism, Learning difficulties and associated disorders.

Our aim is to provide the right support in the right home for all of our service users that we accommodate ensuring that they live their best life.

We provide a high standard of care in an environment that celebrates cultural diversity and promotes equality of opportunity, enabling our service users to feel valued and empowered.

Langley Homes is in Langley Mill, Derbyshire and is in walking distance to local shops and parks and major bus routes.

Job Purpose:

To undertake direct work with our service users as appropriate, attending to their practical, physical, and emotional needs. Aid, support and safeguard our service user’s welfare always, providing excellent care. Work in partnership with other professionals to achieve optimum outcomes.

Main Duties & Responsibilities:

To meet the needs of our service users through:

• Contributing to the formulation and implementation of care and support plans, involving service users and in line with their assessed needs.
• Supporting our service user’s individual development.
• Monitoring each service user’s individual health, with their involvement, reporting any concerns to a senior.
• Working always to safeguard the individuals that we support• Ensuring a healthy, safe and clean environment for service users in their home, encouraging their involvement in achieving this.
• Developing our service user’s contacts with the local community enabling them to integrate and access local facilities of their choice.• Ensure Care Plans and other information about how to support Service Users are followed.
• Supporting our service users in taking prescribed medication as directed in their individual support plan.
• Providing written reports on our service user’s progress and may be required to attend reviews.

To work as part of a team through:

• Strong “can do” attitude.
• Being aware of the aims and objectives of the home and working collaboratively with colleagues to achieve them.
• Protecting service users, staff and visitors from abuse and avoidable harm, including breaches of their dignity and respect.
• Display effective organisational skills at all times.
• Ensuring service users care, treatment and support achieves good outcomes, promotes a good quality of life and is evidence-based where possible.
• Being familiar with our policies and procedures and adhering to them.
• Attending mandatory training and other training identified in personal development reviews to improve work performance and high level of service.
• Assuring the delivery of high-quality person-centred care, supporting learning and innovation, and promote an open and fair culture as part of the Leadership, Management, and Governance of Oasis Care UK Group.
• Participating in regular supervisions with the line manager and attending frequent team meetings and team building exercises.
• Reliable, resilient with motivational attitude.
• Thoroughly documenting relevant information in care and support plans and use communication tools appropriately.
• Complete records as identified i.e. Daily Chronological Logs/ Financial/Maintenance/Health and Safety/Fire/Medication. (This includes daily financial audit checks where applicable).
• Maintaining a high standard of safeguarding for our service users, ensuring code of conduct is followed.
• To carry out any other roles and responsibilities appropriate with the post• Excellent and effective communication skills.
• Great team player at all times• Flexibility to respond to the changing needs of those who we support
